Lack of retaining/gag strap - Defender, Evo/Inspo, HH-CCR, Optima, Liberty, SF2, KISS, Meg etc etc noting only the rEvo and Apoc ship with one!
Lack of BOV - SF2, Liberty (now fixed with a EN250 rated one), rEvo
PPO2 setpoint default of 0.4 - JJ-CCR (now fixed with a software upgrade) Note: not an issue on the HH-CCR, so likely the vibrating HUD may have offset this.....
Two current limited cells - all current recreational eCCRs using voting logic, NOT Poseidon
Miss-assembled (reversible) solenoid - Meg, Based on the NEDU investigation of one.
Both opposed flapper valves screwed together onto one side of a DSV - Hollis Prism2
etc etc

Brad, for the most part, I don't consider the above design flaws. Most of them are user error. Just because someone doesn't follow a check list or can't read the instruction manual, does not necessarily indicate a design flaw. I think you are grasping at straws here.

Furthermore, not using a gag strap can not be considered a design flaw. Sure, it is one approach to potentially solving an issue, but to imply that any designs that do not agree with your personal bias is somehow flawed is a bit arrogant to say the least.

By the way, is that a PDF gag strap that ships with the APOC? I wasn't aware that a PDF provided any critical life saving benefit! :)
 
None the less, it remains the sole source publicly tracking CCR fatalities; I also understand the download rate and repeat visit rate to it is significant....

The recent Skiles case decided liability, which is fine, he didn't buy it from the manufacturer - case closed.

You've got some skin coming on line citing the Skiles case. Why should anyone listen to you after that extraordinary exposition of double standards? For example (and this is just one of many) the manual for your own rebreather states:

USING THIS REBREATHER WITHOUT STUDY AND PASSING A MANUFACTURER-APPROVED TRAINING
COURSE IS NO DIFFERENT FROM TAKING THE CONTROLS OF A HELICOPTER IN FLIGHT WITHOUT
TRAINING: IT IS SUICIDAL.

Then your team tries to blame the rebreather for the death of a diver who was "using that rebreather without study and passing a manufacturer-approved training course".

I have to support rEvo on this, as your not quite right. They have gone against the trend and fitted every rebreather they ship with a good quality Drager gag/retaining strap. Something that stops a diver drowning.... due to being underwater.

Mmmh...it's probably because the unit (a good unit anyway) tend to flood completely in less than 30 seconds if you loose the mouthpiece, without any possibility expel the water???

Simon, As I understand it the rebreather was faultless. It did exactly what it was designed to do!
AFAIK Skiles both studied the unit and was trained to dive rebreathers. He certainly had no issue doing expedition diving on one in Antartica! He also pre-dive filled out a pretty detailed checklist after going through the units function in detail. Not something one can do without education in the matter.
He could have been the world's best IT on that unit and still had the same result.

No, never and in all honestly I just dove the rEvo years ago. I have friends that use this units since years and are very happy.
Beside what one can or can't like it's obvious that a CCR design is a compromise and the manufacturer has his own concepts and ideas.
Anyway for the rEvo the flood is a weakness in respect of other units (which will have other weakness) and the Head-strap will help prevent this problem (among the rest).
Or do you mount now an OPV somewhere? Sorry I'm ignorant about this.
Anyway now HeadStrap is mandatory for CE certified units, whatever the reason.
