My condoleances first! It nearly happen to me one day, for some obscure reasons the O2 bottle was closed and I realized it when checking controller the PPO2 was down to 0.12. That's the day I decided to get a unit with several alarms including vibration and HUD. Believe me I ALWAYS make the same pre dive checks since years and every descending check the controller. Except this day where I got very distracted and stressed because of heaps of people in the water entering zone, which I wanted to escape quickly, scootered at the surface while into the loop and bottle closed. You see this can happen to everyone and I was lucky that no sun dazzled the Shearwater(s) at this crucial moment.

Hi, thanks for sharing your experience. The kind of dives you describe is the most problematic... Distraction, stress, trying to do something in hurry ... most common reasons for fatalityes. Sometimes it is better to say no to the dive when some similar things happen, most problmatic is to spot and acknowlidge distraction, stess...etc

Yess it can happen to everyone, hope everyone is lucky like you when it happens....

Yes, getting in a rush, maybe for a hot drop or something like this or getting distracted by something is bad. There you can miss your best procedures.
I also think this can happen to everyone. Hopefully everyone notice it early enough.
