How an earth have you narrowed the possible causes of the rEvo fatalities down to "if people follow exactly the procedures as lined out, regarding pre-dive and pre-jump checks, cell replacement policy, and if medical issues would not exist at all, there would have been zero rEvo fatalities till now!" when you have not recovered the rEvo's that were being dived nor conducted any accident investigation into the incidents?that is because you just make things up when you have no data!
You know that before blackboxes were incorporated into planes and accident investigations formally done: pilots were blamed excessively as well!

Because one (there are many) of the possible causes for the rEvo fatalities to date may have been the divers inability to quickly bailout to a known safe open circuit gas.
